Review health information for you and your children ages 11 and younger (including blood pressure, allergies, medications, immunizations and more). Securely message your Banner Medical Group, Banner – University Medical Group or Banner MD Anderson provider (messaging is available for most providers). View your lab results, prescribed medications, X-rays and other medical imaging reports. View discharge instructions after a hospital stay or emergency department visit. Getting started is easy. If you already have an account, you can login here using your email address and password. If you need to sign-up for an account, we've made it simple: If you have been seen at a Banner Health location in the past, you can self-enroll using the name, email address and date of birth you used to register for your visit. Note you cannot enroll your children using self-enrollment. You can also have an invitation emailed to you when you are at your next appointment or hospital visit. Just let the staff know you would like an invite when you’re checking in. What is a proxy? A proxy is a person, other than the patient, who has been granted full access to the patient’s MyBanner Patient Portal. This includes parents, Agents as authorized under state law, Guardians and Surrogates for children under the age of 12, and adults who lack decision making capability. You must be 18 years of age or older to create an account. How can I create a portal account? Go to ‘Create an account’ on our website and enter your first name, last name, email address and password. Please provide the same name and email address that you gave at registration at a Banner provider visit or facility. You will need to verify your email address to move forward. Once your information is verified you will be able to log into the portal. How can I access my medical chart? From a Banner Facility: Start the enrollment process at your next appointment or hospital visit. Just let the staff know when you’re checking in. We can send you a secure invitation to access your medical chart. Note that if you are an existing portal user, you will need to give him/her the email you are using on your portal account. Once you receive the invitation in your mailbox from Banner Health Foundation, answer your security questions to access your account. After connecting to the portal, click on the ‘View your medical chart’ tile. Your medical chart will open on a separate tab. From your portal dashboard: If you are a new user: If you have been seen at a Banner Health location in the past, you can self-enroll. Once you are logged into the portal, click on the ‘View your medical chart’ plus button. You will need to enter security questions to access your records. Go to bannerhealth.com, click on Login and fill-out a proxy request form: Request proxy access to your child’s patient portal Request proxy access to an adult’s patient portal Ask our staff at a Banner facility for assistance. If you are eligible, we can send you a secure invitation to access your relative medical chart. Note that if you are an existing portal user, you will need to give him/her the email you are using on your portal account. Once you receive the invitation in your mailbox from Banner Health Foundation, answer your security questions to access your account. Why can't I see my child's information if they are between the ages of 12 and 17? Regulations do not allow you to view certain parts of the medical record for this age group. You may obtain a copy of your child's medical record by contacting our helpdesk at (480) 684-6000 or in-person at any Banner facility. What if I don't get the email to verify my account? Please check your spam and junk folders. Add reply@enewsletter-target.bannerhealth.com as a trusted email address. If you still cannot find the email, you can request another be sent by trying to login using your credentials. A message will display asking you if you want a new email to be sent to you. If you need further assistance please call our support line. Why do you ask for all the information you do when I am creating an account? We require the information requested so that we can match you correctly with your record in our system. Why can't you find my record? The information you provided may be different than the information you gave at registration, or you may not have been seen at a Banner Facility or by a Banner provider. Please email us or call our support line. Accessing Your Account Why can't I log in? Please verify that your email address is correct. If you are using the correct email address, you may need to reset your password. To reset your password, please click on ‘reset my password’ button on the login page. It will redirect you to a new page where you will need to enter your account email address and click ‘Submit’. After this step, an email should be on its way to your mailbox. Click on the email and enter your new password. You will see a success message once your password has been reset. Try to log in again and see if the issue still occurs.
